If they were in the care of a Doctor
If the deceased was under the care of a doctor and has passed away, the GP will issue a Medical Cause of Death document. If the person’s wishes were to be cremated the doctor will also need to sight the deceased and issue a Medical Practitioner Cremation Form. Both of these forms are given to the Funeral Director once the deceased is in their care.
If they’ve passed away at home or unexpectedly
If someone passes away at home and is not under the care of a doctor, or the death is unexpected the police will need to be notified and a decision will be made as to whether the deceased will then be under the care of the Coroner and transferred to Auckland City Hospital. The release of your loved one will be obtained once the deceased has been examined and released to the chosen Funeral Director for transfer from the hospital to funeral home.
It is important to understand that if the deceased is under the care of the Coroner you are not obligated to use the funeral home that completed the initial transfer, as they are contracted to the police for transfers only.
